Three things people tell us on the first call

Most people who land on this page have been managing the same problem for a long time. After hundreds of full-arch consultations, the pattern is fairly consistent — and it almost never starts with "I want implants." It starts with one of these.

"I'm tired of patching."

Another crown has cracked, another root canal has flared up, another bridge has come loose. Each visit fixes one tooth but the underlying problem keeps walking back through the door. All-on-4 is sometimes the more honest answer when the maths on patchwork dentistry stops adding up.

"My denture moves."

Adhesives, sore spots, avoiding steak at dinner, taking it out before kissing your partner — these are the things people don't put in a Google review but mention quietly during a consult. A fixed full-arch bridge changes that day-to-day reality more than any other dental procedure we offer.

"I won't smile in photos."

Not always pain. Sometimes it's just years of avoiding the camera. The aesthetic side of All-on-4 — tooth shape, lip support, midline, gum line — matters as much as the surgery, and it's the part most clinics underweight. We don't.

The procedure, plainly

What All-on-4 actually is

Four implants per arch are placed at angles that take advantage of the bone you already have, and a fixed bridge of teeth is screwed onto those implants. No palate coverage. Nothing comes out at night. The bridge is cleaned around like natural teeth, with a few small differences in technique.

  • Four implants per arch (sometimes five or six in larger jaws).
  • Tilted posterior implants to engage denser bone — often avoiding sinus grafts in the upper jaw.
  • A provisional fixed bridge fitted on the day of surgery for suitable cases.
  • A definitive bridge designed and fitted after the implants have integrated.
  • Long-term review and maintenance every 6–12 months.

All-on-4 is not appropriate for every patient. We'll tell you on the virtual call if your case is more suited to traditional implants, a removable overdenture, or another approach entirely.

Who you'll see

Are you a candidate?

Suitability for All-on-4 isn't decided by a website. It comes down to your bone volume on a CBCT scan, your medical history, the strength of your remaining teeth, your bite, and what you actually want the outcome to feel like. Most Penrith patients send us an OPG before driving in — we'll look at it and tell you honestly whether it's worth your time.

  • You currently wear an upper or lower denture and want it gone.
  • You've been told most of your teeth can't be saved long-term.
  • You've spent more than $20k on patchwork dentistry in the last five years.
  • You have a confident social or work life and your teeth no longer match it.

Three published packages

All-on-4 pricing — per arch

These are the same three packages published on our main All-on-4 page. The difference between Basic, Premium and Platinum comes down to the material and structure of your final teeth — PMMA, acrylic on titanium, or zirconia on titanium. Sedation and extractions are quoted separately because they vary case to case.

$17,999

Per arch · GST inclusive
  • Consultations
  • X-rays / CBCT imaging
  • Surgery with 4 Nobel Biocare implants
  • PMMA All-on-4 temporary teeth with metal support

  • X-rays / CBCT imaging
  • Basic-package surgery with PMMA All-on-4 temporary teeth

Prices shown are per arch. Full-mouth (both upper and lower) treatment is generally double the per-arch package you choose. Sedation and extractions are quoted on top after your CBCT scan, because they vary substantially case to case. Eligibility for early release of superannuation is determined by the ATO or your SuperCare adviser, not by the clinic. Final pricing is confirmed in writing after planning and is held for 90 days.
Anonymised patient cases

Three Penrith-area cases, briefly

Identifying details have been changed at the patient's request. Photos and full case notes are reviewed during your consult. Outcomes vary from case to case and are not a guarantee of results in your own treatment.

Glenmore Park · M, 58 Upper arch

Failing bridgework, 12 years of repairs

Came in with a 14-unit upper bridge that had been recemented three times in two years. CBCT showed enough bone for tilted posterior implants. We extracted the remaining roots, placed four implants and fitted an immediate fixed bridge in the same visit.

Outcome Definitive zirconia bridge fitted at week 16. Patient now eats steak again — specifically mentioned at the 6-month review.
Emu Plains · F, 64 Lower arch

Lower denture wearer, 9 years

Long-term lower denture that had become unwearable after natural bone resorption. The case was on the borderline for All-on-4 versus an All-on-6 with one short implant. We chose All-on-6 in the end — this is the kind of decision the planning conversation actually exists for.

Outcome Fixed lower bridge fitted at 18 weeks. Reported sleeping better within the first month from no longer removing the denture.
South Penrith · M, 51 Both arches

Periodontal disease, accelerated bone loss

Diagnosed with severe periodontitis — teeth were saveable in theory but the long-term prognosis didn't justify the spend. Coordinated full-mouth case planned across two surgical days, four months apart. Included one short upper graft on the right side.

Outcome Both definitive bridges fitted at month 9. Patient is on a 4-month hygiene recall to manage residual perio risk around implants.
Honest comparison

All-on-4 vs traditional dentures

Dentures still have a place. They're cheaper, less invasive and faster to fit. The question is whether you'll actually wear yours every day for the next twenty years — because most people in our experience don't.

Conventional dentures

  • Removable — out at night, in during the day.
  • Bite force: roughly 10–20% of natural teeth.
  • Often requires adhesives once bone resorbs.
  • Upper denture covers the palate, dulling taste.
  • Cheaper upfront, often more expensive over 15 years from relines, remakes, sore-spot adjustments.

All-on-4

  • Fixed — brushed and flossed in the mouth.
  • Bite force: 70–90% of natural teeth.
  • No adhesives, no overnight removal.
  • No palate coverage on the upper arch.
  • Higher upfront, but well-maintained All-on-4 cases routinely last 15+ years.
The bit other clinics skip

When All-on-4 isn't the right answer

We'd rather lose a booking than recommend a procedure that won't work for you. These are the realistic considerations — the ones we'll talk through on your virtual consult before anyone drives in.

You may not be a good candidate if…

  • You're an active smoker and not willing to stop for the healing period — smoking significantly increases implant failure rates.
  • You have uncontrolled diabetes (HbA1c above ~8%). We'll ask you to work with your GP first.
  • You're on certain bone-strengthening medications (bisphosphonates, denosumab). This requires a deeper conversation with your prescriber.
  • Your remaining natural teeth are healthy, well-aligned and well-restored. Don't fix what isn't broken.
  • You're hoping All-on-4 will solve a TMJ or chronic headache problem — it won't, on its own.

Realistic risks to know about

  • Implant failure rates are low (around 1–3% in healthy patients) but not zero. We replace failed implants under our written warranty.
  • Day-of-surgery swelling and bruising for 3–7 days. Most Penrith patients take a long weekend off work.
  • Soft food diet for the first 8–12 weeks while implants integrate.
  • Speech adjustment in the first 2–4 weeks — the bridge feels different from a denture.
  • Long-term hygiene is non-negotiable. Skipping reviews leads to peri-implantitis, which is harder to treat than gum disease.
Written quote, no surprises

What you'll see on the quote

After your CBCT scan you receive a written treatment plan with the package price you've chosen, plus the sedation and extraction line items separately so you can see how the total comes together. Nothing is added the morning of surgery. If your scan shows the case isn't suitable, we'll tell you that on the planning visit.

Penrith → Erskineville journey

Four stages, designed around the drive

Erskineville is a 45–60 minute drive from Penrith via the M4. We don't hide that. We do try to make sure you only make the trip when it's clinically worth it.

1
Week 0 · From home

Virtual consult

20–30 minute video call. You send any existing OPG or CT scan in advance. We talk through the problem, look at imaging together, and give you an honest read on whether All-on-4 is the right path. If it isn't, we tell you that on the call — no in-person trip needed.

2
Week 1–3 · One trip in

CBCT scan, digital impressions, written quote

Your first in-person visit. 3D CBCT, intra-oral scanning, smile design conversation, photographs, bite analysis. You leave with a written treatment plan and quote within five business days — usually before you've made the drive home.

3
Surgery day · One full day

Extractions, implants, immediate bridge

IV sedation if you've chosen it. Extractions, four (sometimes five or six) implants placed, and a fixed provisional bridge fitted on the same day for suitable cases. You leave with teeth that don't come out. Most Penrith patients arrange a driver and stay locally for one night.

4
Months 4–6 · 2–3 trips

Definitive bridge & sign-off

After implants integrate, we replace the provisional with the definitive zirconia bridge. Final smile design refinements, occlusion balancing, photographs, and a long-term maintenance plan. You're then on a 6-monthly hygiene recall.

Common questions

All-on-4 Penrith FAQ

These are the questions we actually get on the first call — in roughly the order they come up.

Is it really worth driving from Penrith to Erskineville for this?
Honest answer: only if the case is suitable and you want a more involved planning conversation than what's on offer locally. We get patients who've had three or four consults in Western Sydney before driving in. We also get patients we send back to a closer dentist after the virtual call because their case doesn't need full-arch treatment. The virtual consult exists so you don't waste a Saturday on a trip that wasn't going to lead anywhere.
How much does All-on-4 actually cost in Sydney?
At our clinic, All-on-4 is offered in three published packages per arch: Basic $17,999, Premium $27,998, and Platinum $32,998. The price difference between tiers is the material of your definitive teeth — PMMA, acrylic on a titanium frame, or zirconia on a titanium bar. Sedation and extractions are quoted separately because they vary so much case to case. For full mouth, you'd typically double the package price you choose. Final figures are written into your treatment plan after the CBCT scan and held for 90 days.
Can I get same-day teeth?
For most suitable cases, yes — you leave the surgery with a fixed provisional bridge already in place. Whether your case is suitable for immediate loading depends on your bone density, the primary stability we get on the implants on the day, and your bite. We'll know within the first hour of surgery. If for any reason immediate loading isn't safe, we'll fit a temporary fixed-removable transition instead.
Do All-on-4 implants hurt?
The surgery itself isn't painful — you're under local anaesthetic and, if you've chosen it, IV sedation. The discomfort is in the first 3–5 days afterwards: pressure, bruising, mild swelling. Most patients describe it as much less painful than they expected and a fraction of what wisdom tooth surgery felt like. Standard pain relief manages it.
How long does the whole process take?
From the first virtual consult to the definitive bridge being fitted: roughly 4–6 months. The surgery is one day. The wait between immediate bridge and definitive bridge is for healing — the implants need to integrate with bone before the final restoration is loaded. Some complex cases take longer.
Will I need bone grafting?
Often no. The whole point of All-on-4's tilted posterior implant geometry is that it engages denser bone further forward, frequently avoiding sinus grafts in the upper jaw and steep bone augmentation in the lower. That said, around 1 in 5 of our cases needs some grafting — it's identified on the CBCT scan and quoted up-front, never added later.
How long do All-on-4 bridges last?
Long-term studies on the original All-on-4 protocol show implant survival above 94% at 10 years in well-maintained cases. The bridge itself (the part you see) is typically replaced or refurbished at around 10–15 years — like any prosthetic, it wears. The implants underneath are designed to last decades when properly looked after with hygiene visits.
Am I too old for this?
Age on its own doesn't disqualify anyone — we've placed full-arch cases in patients in their late 70s. What matters is general health, healing capacity, medications (particularly bisphosphonates), and your daily ability to maintain hygiene around the bridge. The CBCT and a proper medical history conversation tell us far more than a date of birth.
Can I use my superannuation?
Some patients access early release of super on compassionate grounds for major dental treatment, through services like SuperCare or by direct ATO application. Approval is at the ATO's discretion based on your circumstances and is not something we control. We can provide the supporting clinical documentation; the financial application sits with you and the relevant provider.
What if I'm a smoker?
Smoking measurably increases implant failure rates — roughly doubles them in some studies. We don't refuse to treat smokers, but we'll have a direct conversation about it and we ask for cessation (or significant reduction) for at least the surgery and integration period. Vaping has the same wound-healing concerns and is treated the same way.
How do I clean it?
Like teeth, with a couple of additions. Standard brushing twice a day, plus a water flosser (we recommend specific brands), interdental brushes for under the bridge, and a 6-monthly professional clean. The bridge can be unscrewed and serviced if needed at the 5-year mark, though most patients never need that.
What's the warranty?
We provide a written warranty at the time of treatment covering the implants and the bridge separately, with terms tied to your hygiene maintenance schedule. The exact wording is in your treatment plan. Manufacturer warranty on the implant fixtures themselves is typically lifetime under normal use.
